Your Lab Requisition Form
Once your insurance is approved for genetic testing through our lab, we will send a requisition form to your healthcare provider. This form will contain an Apollo Genetics logo in the top left corner, your information, and 3 small sections for your provider to complete. These sections include choosing a panel in section 5, filling in medical code(s) in section 6, and their signature in section 10. Our provider integrations teams will follow up with your provider's office to help them understand how to fill out the form. In most cases, they will have the form ready to send back within a week or two. By reaching out to them with a quick phone call to inform them of your interest in your test, you can speed up this step in the process significantly. As soon as we receive the completed form back, we will ship your testing kit directly to your home.
